This is an open challenge for everybody to work on and build on but it is also the overall challenge for a collaboration – and a Design Thinking Competititon – between The Health Movement, Karolinska institutet and Openlab.

The Health Movement has received funding from EIT Health to run a Project for two years, with start in January 2017. The project goal is to develop the Health Movement, a dynamic ecosystem of community actors to prevent chronic diseases like for example diabetes type 2. The impact is lowering the risk of developing lifestyle related chronic diseases and decreasing costs with preventive interventions. The main objectives are:

• Integrate existing wellness and health services to create seamless network of health service touch points and ‘nudges’ in the citizen’s daily interactions.
• Establish a method for screening undiagnosed patients to localize service needs. For example The International Diabetes Federation estimates that 193 million people with diabetes are undiagnosed.
• Create new tools to share risk and value between different partners within the health ecosystem using the financial tool of Health Impact Bond to move the focus to the value and impact of the investment. The metric of the contract is a health index defining the health outcome of the target population - the goal is to procure a defined, measurable and valued health.

The need to identify the target population

Strategies tailored at increasing levels of physical activity depend on the population at risk; age, cultural and socio-economic background, as well as physical and psychological status, need to be carefully taken into consideration to determine which strategies can the most effective.

In the long term, education and highly engaging programs on exercise as part of extracurricular activities might constitute the most cost-effective alternative to reduce obesity and life-style degenerative diseases. Thus, targeting children and teens should be the priority.

On the other hand, if a particular, different population, i.e., young adults or the elderly, has already been identified (in Sweden?) as being at risk of developing obesity or type 2 diabetes due to lack of physical activity during the next decade, these groups should be targeted first. By the same token, culture and/or religion might dictate access and frequency of participation to sports facilities and/or in sports programs.

I believe that the metrics identifying success (or lack thereof) of a program aimed at increasing levels of physical activity can be better evaluated once a particular target population and concrete goals from the program (BMI, insulin sensitivity, engagement in exercise programs, among many others) are defined.
